General Summary
The Transition Advocate will conduct outreach to inform Community Based Organizations, Community Members, and institutional facilities about the transition services ILRC provides to individuals with Traumatic Brain Injuries (TBI) in San Luis Obispo County. The Transition Advocate will provide enhanced information and assistance, options counseling and person-centered planning, short-term service coordination, and transition services in Santa Barbara County and will support these efforts in Ventura County, while prioritizing outreach to individuals with TBI’s. The Transition Advocate will focus on transitioning individuals with TBI’s from:
- Hospital to home
- Nursing facility to home
- Intermediate care facility for individuals with intellectual and developmental disabilities to home
- Any other institutional settings to home

About the Organization
The Independent Living Resource Center (ILRC) is a disability focused non-profit providing services and advocacy to those experiencing all disabilities, of all ages, and all income levels. Our mission is to promote independent living and full access for individuals with disabilities through advocacy, education and action in our communities. The organization has been based in Santa Barbara since it opened in 1976 and serves the Central Coast Counties of; Ventura, Santa Barbara, and San Luis Obispo. Services are provided by utilizing a team approach where the consumer is an active member in their goal attainment. Services provided by the organization include; Housing, Personal Assistance, Interpreting, Peer Support, Assistive Technology, Youth Transition, Older Individuals who are Blind Program, Advocacy, and Emergency Preparedness. ILRC is actively engaged in all of our counties.
Job Duties & Responsibilities
Responsibilities:
- Develop a comprehensive community outreach plan to inform the public about the ILRC’s transition services for individuals with TBI’s
- Conduct outreach to hospitals, nursing homes, and other medical and residential facilities to identify individuals with TBI’s who want to transition back the community
- Conduct consumer intake procedures and needs assessments
- Manage all referrals and refer to appropriate staff/agencies using no wrong door policy
- Using a person-centered approach, interviews consumers to ascertain immediate and long-term needs for services
- Coordinates transitions from medical and residential care facilities
- Draft written Independent Living Plan goals with all consumers or obtain waiver and monitor, follow-up on consumers’ progress
- Record consumer demographics and service activities into necessary databases as part of the monitoring and evaluation process
- Update documents an services as provided and on a timely basis in documentation databases
- Advocate with consumers when access rights are limited or denied
- Provide initial transition information, options counseling, person centered planning transitional care plans
- Refer consumers to resources and support programs that will enhance their independence
- Coordinate with other ILRC staff to assist transition participants with IL services
- Facilitate the maintenance of housing resource information for referral to consumers and assist consumers in their application process to available housing programs
- Refer consumers to resources and support programs that will enhance their independence
- Compile and provide requested data for reporting purposes as needed
- Serve on Committees to promote Systems Advocacy in the areas of housing, transportation, and Personal Assistance and keep in the know regarding area issues
- Utilize and follow ILRC policy, procedure and service provision manuals
- Participate fully as a member of a working team and promote harmonious working relationships
- Conduct agency business in a professional manner
- Other duties as assigned
Required Qualifications
- Have and use excellent communication and organizational skills
- 2 or more years’ experience in related field
- Experience working with persons with disabilities
- Familiarity with Skilled Nursing Facilities
- Ability to meet deadlines, balance multiple tasks
- Ethical work practices: honest, dependable, team-oriented work style.
- Ability to arrange for/provide own transportation throughout the service area.
Preferred Qualifications
- Personal experience with disability
- Bilingual English/Spanish or English/ASL
