ASL Interpreter Request Form
To schedule an ASL interpreter appointment, we will need this information at least 72 hours in advance. Due to a shortage of interpreters in this area we recommend getting your request in as soon as possible. We will try to do what we can for last-minute requests.
NOTE: For doctor's appointments, sometimes the deaf parents are the ones who need the ASL interpreter, so it is helpful to get the patient's name as well. It helps the interpreter check in at the front desk.
